Most braces problems are not true emergencies. A loose bracket, a poking wire or a lost elastic can wait for office hours with a little wax and care at home. Call right away for facial swelling, trauma to the teeth, or pain you cannot control.
What counts as a real orthodontic emergency?
The word emergency does a lot of unfair work in orthodontics. It is worth separating the handful of situations that need same-day attention from the much longer list of annoyances that do not.
Call us immediately
A blow to the mouth that loosens or displaces a tooth, swelling of the face or gums, uncontrolled bleeding, or a wire that has been swallowed all need prompt attention. If the injury involves the teeth themselves rather than the appliance, you may need a dentist or an emergency room first, and then us.
Call during office hours
Loose or broken brackets, a wire that has slid out of place, a poking wire end, a lost separator, a broken elastic hook or a band that has come off are all appliance problems. Handle it at home
General soreness after an adjustment, a mouth ulcer where a bracket rubs, or a tooth that feels loose while it moves are normal parts of treatment. They need comfort measures, not an appointment.
What should you do about a poking wire tonight?
A wire end that works its way into the cheek is the most common after-hours complaint, and it is also the easiest to manage yourself.
Cover it first
Dry the area with a tissue, pinch off a pea-sized piece of orthodontic wax, roll it into a ball and press it over the sharp end. Wax will not stick to a wet bracket, which is why most people think the wax does not work. Replace it after meals.
Tuck it if wax is not enough
The eraser end of a clean pencil or a cotton swab can gently push a protruding wire back toward the tooth. Move slowly and stop if it resists. If a long wire end is unbearable and you cannot reach us, sanitized nail clippers can trim it behind the last bracket, with gauze in place so the piece is not swallowed. Only do this as a last resort, and tell us at your next visit.
Soothe the sore spot
Rinse with warm salt water a few times a day, keep eating soft foods and give the ulcer a day or two. The tissue in your cheek toughens quickly, which is why this happens far less often after the first months.
What do you do when a bracket comes loose?
Brackets are bonded to enamel with adhesive, and hard or sticky food is what usually breaks that bond. Nothing dramatic happens next, but the tooth stops moving until the bracket is back on.
If it is still on the wire
Leave it there. Slide it back into position between the neighbouring teeth if it has spun around, cover it with wax if it irritates your cheek, and call us to schedule a repair. Do not try to re-glue it yourself.
If it has come off completely
Keep the bracket in a small bag and bring it with you. Removing a rear bracket or band that is off the wire is fine; save the piece so we can check whether it can be reused.
Does a loose bracket delay treatment?
A single one, fixed promptly, usually costs very little. A pattern of them is different: each break pauses the movement on that tooth, and repeated repairs are one of the most common reasons a case runs past its estimate. If yours keep breaking, tell us, because the fix may be a different bracket or a change in diet rather than better luck.

What about appliances other than braces?
Brackets and wires are not the only things that can break, and the rules are slightly different for each appliance.
Expanders and other fixed appliances
If an expander comes loose on one side, stop turning the key and call us. Continuing to turn a partly loose appliance can make it harder to re-cement. Keep the key somewhere you will find it again and bring it to the repair visit.
Lost or broken retainers
Collect the pieces, do not try to repair them yourself, and call us promptly. Teeth begin drifting within days of a retainer going missing, so a lost retainer is more time-sensitive than a broken bracket even though it hurts less.
Elastics and separators
A lost elastic is replaced from your own supply; a lost separator usually means the space has already opened, but tell us so we can check. Neither needs an urgent appointment.
What belongs in a braces emergency kit?
Everything on this list fits in a pencil case, and keeping one in a backpack or glove box turns most after-hours problems into a two-minute fix.
The essentials
Orthodontic wax, a small mirror, tweezers, a travel toothbrush and floss threaders, sanitized nail clippers, salt for rinses, and spare elastics if you have been given them. Add our number, (850) 385-8101, to your phone so nobody has to search for it at 9 PM.
What to leave out
Superglue, household pliers and anything meant for a different job. Repairs that seem clever at midnight tend to cost an extra visit.
How do you avoid braces emergencies in the first place?
Most broken brackets trace back to the same short list: ice, hard candy, nuts, popcorn kernels, crusty bread, whole apples and carrots, and chewy caramel. Cut firm foods into pieces, skip the ones that cannot be cut, and wear a mouthguard for contact sports. Breakages cluster in the first weeks of treatment, before the new habits settle in, which is worth knowing if you are starting braces as a teen. Younger patients in braces for kids do best when a parent does a quick check of the lunchbox.
How do you manage the soreness that is not an emergency?
Discomfort after an adjustment is expected, predictable and short. Knowing the pattern keeps people from treating it as a problem.
The first 48 hours
Teeth feel tender to bite on for a day or two after the wire is changed, and most strongly after the very first wire. Soft foods, cold water and a salt-water rinse cover most of it. Use whatever over-the-counter pain reliever you normally take, following the label.
Cheek and lip irritation
New brackets rub until the tissue toughens. Wax over the offending bracket and a salt-water rinse are the whole treatment, and the problem largely disappears after the first weeks of braces treatment.
When soreness is not routine
Pain that grows rather than fades, throbbing in one tooth, or swelling around a gum are not adjustment soreness. Call us, and if the tooth itself is the source you may need your general dentist as well.

Can I go to a regular dentist for a broken bracket?
Keep seeing your dentist for cleanings and cavities, but appliance repairs belong with your orthodontist. Reach out to us and we will get you in.
Should I cut a poking wire myself?
Only as a last resort when you cannot reach us and the discomfort is severe. Wax first, clippers second, and tell us what you did at the next visit.
How fast can I be seen for a repair?
We keep room in the schedule for repairs. Call or text as soon as it happens and we will find the soonest slot; our hours are Monday through Thursday, 8 AM to 5 PM.
Is it normal for teeth to feel loose during treatment?
Yes. Teeth loosen slightly as they move through bone and firm up again as the bone rebuilds around them. Tell us if a tooth becomes markedly loose or painful.
